Filing a complaint

 

To file a complaint, follow the steps below:

​ 1. Return/complaint form Fill in the return/complaint form and attach it to the shipment. The document should contain the following data: name and surname, e-mail address or correspondence address, telephone number, product number and reason for complaint, date of purchase and occurrence of the defect.

2. Returning the product When filing a complaint, please return the product together with the receipt or the invoice note issued by the customer if the sale was on an invoice.

3. Shipping The product under complaint should be sent to the following address: Biolive Innovation.pl ul. B. Dobrzańskiego 3 20-262 Lublin NIP 946 265 65 67

 

Return of goods

 

According to applicable law, you have 14 days to return the goods. To withdraw from the contract and return a product purchased from us, complete the required declaration and send it to our address together with the returned product/products: ​ Biolive Innovation.pl ul. B. Dobrzańskiego 3, 20-262 Lublin NIP 946 265 65 67 ​

Also attach the receipt or invoice note to the package with the declaration and the returned product. Failure to provide the receipt or invoice note prevents the completion of the return/complaint process.
